How to create AVC/MVC elementary stream ?

Hi,
I'm going to try MakeMKV and I would like to know if it could be possible to know how MakeMKV merge AVC and MVC in an elementary stream ?
Is it an external tool or a code write by you ?

Any idea ? I would like to understand how does MakeMKV work with MVC.
