The disc must be detected by the OS (Windows), after this MakeMKV can access the disc.

What happens if the first DVD (the that works) is inserted?

Something has changed. Don't think so, no problem at all with subtitles. Even if they are in extra tracks or with the "Forced flagged" subtitles. Incorrect aspect ratios have nothing to do with DVDFab or MakeMKV. This are mastering errors of the DVD. The aspect ratio is set in the IFO file and in the stream. Can be done with "MKVToolNIX GUI". Go to the output tab and select "Before chapters" and add "all" or specific Chapter numbers.

Never used this PiP feature, but form my experiance I would say, this is included in an subtitle stream. I have seen subtite tracks during test and verifiy with pictures (still frames, no moving video). Subtitles are an overlay picture (text or whatever).

As far as I know, DLNA (UPnP) supports only VOBSUB (DVD) and not HDMV/PGS (Blu-ray) subtitles. Solution is to convert the HDMV/PGS subtitle(s) with the Java tool "BD Sup2Sub".
