Figures. The tool I've used in the past was from VSO software (ConvertXtoDVD), but IIRC, until it's paid for, it also has the 'trialware watermark'. 7 day trial... give it a shot. There must be a usable freebie out there, somewhere.

A log would help immensely, but the advice will probably be to try cleaning the disc... despite it appearing pristine to the human eye.

This appears (FW date) to be a SVC NS40 drive, manufactured prior to late 2015. NOT UHD capable.

LibreDrive is a 'work in progress' & overriding FW enforced speed control is planned... 'Unrestricted read speed: Possible, not yet enabled' currently reported for all drives I've seen, so far. When implemented, this status will change. There are 2 hardware versions of BH16NS40. NS50 (UHD friendly) ...

Just a guess, but did you happen to change the 'MakeMKV data directory' (General prefs tab) from the default of 'C:/Users/%USERPROFILE%/.MakeMKV' to a location you have no write permissions to? 'C:\Program Files (x86)\MakeMKV', perhaps? Aside from these crashes, you'd also lose user config data, etc...
