I think MakeMKV (V = Video) select only "video" titles and not picture or pure audio (MKA instead of MKV) titles. Try to use the MakeMKV backup function and check the decrypted backup for special files outside the STREAM folder.

To rip something from title 3 you must specicfy "3:1-5", where 3 is the title and 1-5 means chapter 1 to 5. To find out which chapters you can use VLC (Playback-> Chapters).

Just tested, extract to ".makmkv", rename the file, changed the options and name in the file and the "My Default" xml set is shown in the advanced tab.

If many discs could not be read, I would say it is a drive problem. In your log you see "Drive Timeout" as a result, that the data could not be read from the disc and the drive tries it many time until the timeout message from the OS is shown.

CC (Closed Caption) subtitles needs the external utility "ccextractor" to convert it to text subtitles.

Only the MakeMKV support (Mike) knows the meaning of the Debug Codes. Try it with deselecting all subtitle tracks, maybe it help. But anyway, really ... 4:3 Letterbox?
