Since the BD itself is made up of multiple files, the only way to get "everything" in "one file" would be to rip it to an ISO image. Then, you would need a BD-aware player to be able to decrypt the files within it when you play it. MakeMKV is going to either pull the tracks as MK...

What version of Handbrake are you using, the release version, or one of the nightly builds?

Lots of possible problems, from bad/dirty disks to bad/dirty drives to, yes, bugs.

If the output is MP4, then yes, you can only do one track, and it is burned in...
But, if the output is MKV, handbrake will copy the PGS subtitles to the resulting MKV file, and (assuming player support) they are all available.

"Always on" is a function of what you play the files with. The Uebo M400 always plays subtitle track 1, until you turn it off. Their M100, on the other hand, never plays subtitles, until you turn them on. VLC goes by your preferences, but the default is to play the first track of your pref...
