Help with Grainy Videos

Recently got into creating MKV files of all my BluRays and UHD BluRays in hopes that I can utilize Plex or Kodi (or something ) to stream from my Synology NAS.
I am running into an issue though and I have no clue how to fix it. My understanding is when you use makemkv to ripe a BR there is no transcoding or encoding that happens. It is a straight copy into the mkv container. But when I take a movie and compare it to the Blu Ray version (swapping inputs back and forth OR even using the same device to play the MKV that I use to Play my BluRays the MKV files are good but not identical, I see a lot of grain an what looks like MPEG noise.
I seem to notice it on HDR content more than SDR but it is still present on everything.
I would post a picture but it is near impossible to visualize it but I can try.
One Title I notice is a Lot is on the 1080p Avatar BluRay. The BluRay is perfect and the MKV looks good but not the same
