Your drive is having trouble reading parts of the disk. And, while it tries REALLY HARD to read it, the operating system decides it is off-line.
Step one is to try cleaning the disk. Yes, even if it looks "pristine", "perfect", and "just came out of the package". See the sticky at the top of this forum about "read this if you are experiencing read errors"; inside, there is a link to a site that explains one of the better ways to clean disks.

Success! I was finally able to read the (23 GB) file using good 'ol Windows Explorer. It just took a .. really .. long .. time.
I tried raising the number of retries in MakeMKV, but it timed out and failed.
I guess I'm that guy
