I've had a repeated failure with title 2 (with chapters) and title 7 (without chapter). This is episode 2. I was able to create a .mkv file for episode 1 without a problem from the same disk. Here is the error report...

To me, that clearly says that it cannot read the contents.

My discs were doing this as well. They looked perfectly OK, had cleaned them with a soft cloth. Then I remembered what Mike always says: just because they look clean does not mean they are clean; the production process can leave a very thin layer on the disc, clean them with mild soap.

So I thought; won't hurt. Cleaned the disk with warm water and some mild soap, dried them off with some soft tissue and lo and behold! Now it copies over with no problem. One of the discs before would not even mount. None of my players recognized it, not it works fine.
